Severe lower extremity ischemia is a disease caused by blockage of major blood vessels that supply blood to the legs. As peripheral arteriosclerosis worsens, blood flow gradually decreases, resulting in leg tissue necrosis and amputation. Angioplasty such as stent insertion and anti-thrombotic drugs are used for treatment, but there is a risk of blood vessel damage or recurrence of blood clots, so there is great interest in developing stem cell therapy.

However, stem cell treatments have a good tissue regeneration effect, but when administered alone, there is a problem of low cell survival due to hypoxia and immune response at the damaged area. To increase cell survival, it is necessary to develop a delivery system that uses biodegradable polymers or extracellular matrix components as a support.

In this study, the research team made a hydrogel from biocompatible materials, collagen and hyaluronic acid, and processed it into micro-scale to create collagen microgel, a porous three-dimensional tissue that is easy to transplant into the body and has uniform cell distribution. Subsequently, adipose-derived stem cells were cultured in collagen microgels to complete a microgel-cell self-assembly stem cell treatment product.

The research team found that collagen has excellent biocompatibility and cell activity, so it can induce self-assembly of cells by promoting the interaction between microgel particles and collagen receptors in stem cells, and the gap between microgel particles increases the porosity of the three-dimensional tissue. It was explained that it increases drug delivery efficiency and cell survival rate.

When the microgel-cell self-assembly stem cell treatment developed by the research team was administered to a mouse model of severe lower extremity ischemia, more angiogenic factors were discovered than when the stem cell treatment alone or a simple mixture of stem cells and microgel was administered. It was found that the blood vessel regeneration effect was much higher.

The microgel-cell self-assembly stem cell treatment had a 5 to 10 times higher blood flow and blood vessel regeneration rate than when only stem cells were administered, and the blood flow was about 40% higher than when a simple mixture of stem cells and microgel was administered. , the rate of lower limb muscle and blood vessel regeneration was found to be increased by 60%.

The research team said that the newly developed stem cell treatment showed excellent angiogenesis effects and could be a new alternative for patients with severe lower extremity ischemic disease who had limited treatment options other than amputation, and that it could be applied to other diseases with mechanisms similar to peripheral artery disease. He said he could do it.
